To address the needs of the growing population of Hepatitis C patients in DC, Grubb’s offers a special support group called HepC and Me.

The focus of HepC and Me is to provide a comfortable, safe environment for patients to share their struggles, ask questions, find support, and make friends with others going through the same treatments. Grubb’s NW offers:

The HepC and Me program is available to anyone diagnosed with HepC seeking treatment and care. The program components are as follows:

NAVIGATOR TO PILOT:
Participants are supported from the start of treatment to the end by HepC mentors who have been cured. Our mentors want to provide hope and help participants navigate through their treatment.


LIVE:
Citywide support groups for those diagnosed with HepC and are considering or are currently on treatment. Click here to see upcoming events.


LOVE YOUR LIVER:

EACH ONE, TEACH ONE:
Individual and group HepC education surrounding the disease, testing, diagnosis, treatment and side effect management.


EAT TO LIVE:
Participants are given a convenient, pocket-size nutritional meal guide to help them understand the nutritional requirements necessary for effective therapy of certain medications used to treat HepC. Click to view the Eat to Live page.


ONE DAY AT A TIME:
A patient adherence program that phones or sends text messages to those patients on treatment as a reminder to take HepC medications as prescribed.


LINX TO CARE:
A treatment referral program identifying and linking patients diagnosed with HepC, lost to care, or previously unsuccessful in their treatment to local HepC providers and clinics.


INJECTION TRAINING:
A nurse, pharmacist, or medical assistant will provide free injection training in a comfortable and private room at our pharmacy location.


RED CARPET SERVICE:
A free, supportive transportation service that provides patients door-to-door service to their designated HCV medical care team or provider. Patients are identified by the medical care team (provider, nurse, or medical assistant). This service targets patients who are at “high risk” for noncompliance to doctor appointments and/or treatment. HepC and Me mentors pick up patients, transport them to their doctor appointment, wait, and then returns the patient to the comfort their home.


TELE-SUPPORT GROUP:
The free HepC and Me tele-support group meets every third Wednesday from 6:30 PM to 8 PM via telephone.The support group is facilitated by several cured HepC patients that share their experience, strength, and hope while living with HCV and their journey to a cure. They assist in providing psycho-social support to those newly diagnosed, those considering HCV treatment, or those struggling to remain on HCV treatment.
